Registration Information
2008-2009
CHILD CARE RATES:
Morning Care Rates: $3.00 per day or $15.00 per week (7:00a.m.-8:30 a.m.)
TK-2nd Grade After School Rates: $2.00 per each hour of care
(Sibling Discount: 25% off of day care charges for the least expensive child.)
LATE PICKUP (After 6:00 p.m.) $1.00 per minute or any parts thereof
$25.00 fee upon second occurrence and thereafter + $1.00 per minute
EMERGENCY MEDICAL FORM:
This form must be on file in our school office to allow your child to attend the extended program.
HEALTH RECORDS:
All health records must be on file in our school office to allow your child to attend the extended program.
AFTERNOON SNACK: Parents are responsible to pack their child a healthy afternoon snack each day that they will be remaining for extended program. Clearly mark the items for “extended snack” for your child and their teacher. Water or juice will be provided by the extended program.
EARLY RELEASE DAYS: (Students should pre-register for care on these days.) A healthy lunch should be sent with your child, along with an afternoon snack if staying after 3:30 p.m.
11:45 a.m. dismissal: $2.00 per hour for each hour requested (pre-registration required) - or in attendance
PAYMENT INFORMATION: All charges will be billed on a weekly basis for child care services. They must be paid on a weekly basis. Late payments will incur after one week, a charge of $20.00 with be applied to your account. Your account must be in good standing at all times to be permit them to attend the extended care program.
Save your receipts for income tax purposes. Please request a year-end statement through the school office.
SCHOOL UNIFORMS:
School uniforms are required for all grades from Kindergarten-2nd Grade.
When buying uniforms please pick-up required list from school office. Students in grades TK – 2nd grade are required to wear uniforms daily. Students should purchase matching jackets, sweaters, or zipper sweatshirts for use in colder weather. Shoes should be rubber soled, closed toed and closed heels for the sake of safety.
